Your 5‑Minute Med Checkup: What Your Answers May Mean
Thanks for taking a moment to be honest about how you’re doing. This page is for education only. It can’t diagnose you or tell you to start, stop, or change any medication.
If you are in crisis or feel unsafe, please call 988 or go to the nearest ER.
Where you might fit…
Most people who complete this checkup fall into one of these patterns. See which sounds most like you:
1. “Doing okay, but not quite yourself.”
Your symptoms bother you sometimes, but you’re mostly getting through the day. You might benefit from fine‑tuning meds, side effects, sleep, or routines.
2. “Still really struggling.”
Your mood, anxiety, focus, or sleep are often getting in the way of work or relationships. That usually means your current plan isn’t a great match yet, not that you’ve “failed.” Lots of people in this spot benefit from a more thorough medication review and sometimes adding therapy.
3. “Feeling overwhelmed or stuck.”
Your symptoms and/or side effects feel severe, and most days are a grind. That’s usually a big signal to get professional support and not try to white‑knuckle it alone.
Who we can help…
At Citywide Mental Health, we can currently help adults 18+ in California who:
Have Aetna, Anthem Blue Cross, Cigna, UnitedHealthcare/Optum, or are open to paying out of pocket or using their PPO benefits
We’re not able to bill Medi‑Cal or Medicare at this time.
Next step: Free Care Options Call (about 15 minutes)
Schedule a brief call with our Citywide Mental Health Care Team (not a clinician) to:
Confirm whether we’re in‑network with your insurance or discuss cash options
Make sure your situation fits what we do best
Answer logistical questions about starting care
If it’s a fit, book a full telehealth evaluation with our nurse practitioner
We don’t give medical advice or change medications on this call. It’s simply to see if we’re the right home for you.
If you’re outside California, have Medi‑Cal/Medicare, or we’re not a fit, we’re still glad you took this step. You can:
Contact your primary care clinician
Search for local providers through your insurance website or Psychology Today
Call 988 or go to the nearest ER in an emergency
